Our Doctors

Richard Kootman, M.D.

Richard Kootman, M.D., founder and Medical Director of the Arrowhead Eye Center, is a Board Certified Ophthalmologist, Corneal Specialist, and a Fellow of the American Academy of Ophthalmology. Dr. Kootman graduated from Tulane University, and went on to receive his Medical Degree from LSU School of Medicine. He did his residency in Ophthalmology atthe Tulane Medical Center, and furthered his experience with fellowship training in corneal disease, corneal transplantation, and refractive surgery at the St. Louis University Medical Center. He is member of the American Academy of Ophthalmology, Phoenix Ophthalmologic Society and Arizona Ophthalmologic Society. As an experienced anterior segment surgeon, Dr. Kootman was the first in Arizona to perform a newer technique in corneal transplantation called, DSEK. He specializes in complex cataract surgeries, corneal transplants, as well as, LASIK Vision Correction. He is a clinical instructor at the Midwestern School of Medicine in Phoenix, Arizona, and frequently lectures on cataract and refractive surgery.

Dr. Kootman and his wife, Jettie, have three children, Brooke, Drew, and Michael. He enjoys golf, magic, running, and stock trading.

Tara Miller, O.D.

Dr. Tara Miller, O.D., MBA earned her undergraduate degree in Biology and Chemistry at University of Mary in 2004 located in Bismarck, North Dakota and is a graduate of Southern California College of Optometry, Fullerton, CA. After completing her Doctorate of Optometry, Dr. Miller continued her education to receive a Masters degree in Business Administration with an emphasis in Health Care from the University of Mary in 2011. She has been practicing in the Phoenix valley for four years and specializes in refractive and medical conditions including comprehensive vision exams for patients of all ages. She has extensive training in treating eye diseases, prescribing eyewear, fitting contact lenses, and co-managing surgeries, including cataract surgery and LASIK. She has experience treating a broad range of visions disorders and conditions.

Dr. Miller conducted her clinical training at Tucson’s Northwestern Veterans Affair Clinic treating and managing eye diseases in our veterans, working in Las Vegas at Wellish Vision Institute which specializes in refractive surgeries and glaucoma care, and Fort Yates Indian Health Services Clinic in North Dakota. She enjoys pursuing continuing education in her areas of specialty and is a continuing member of the American Optometric Association, American Optometric Society, and Arizona Optometric Association.

A native North Dakotian, Dr. Miller grew up in the rural Midwestern community of Crosby, ND. In this farming and ranching community, Dr. Miller learned the work ethic so many midwesterns pride themselves on. She takes pride in providing a high quality exam in a personal and friendly manner, allowing an overall positive experience for her patients. In her free time she enjoys running, hiking the nearby desert mountains, spending time with her two chihuahuas, and playing Texas Hold 'Em Poker.
